First Aid Credit Transfer Request Form

This form is to be completed by individual SLSSA Club members, or by someone on their behalf, such as a parent, Club Trainer or Chief Instructor. 

• You will need an electronic copy of the qualification or Statement of Attainment you are seeking Credit Transfer for, as you are required to provide a copy with this submission.  

• Only qualifications that are still in date and current, will be considered for Credit Transfer 

Please note:

• In line with RTO Compliance requirements, SLSSA will validate all qualifications and Statement of Attainment, by checking the validity of each certificate with the issuing RTO. 

• Only Units of Competency that are still current can be considered for Credit Transfer.
