Probationary Trainer Supervision Form

Supporting & Developing Volunteer Education Teams, Member Education

    1. Provide the required information regarding the people involved, the Surf Award being delivered.

    2. This form is to be used throughout a course to capture feedback during different stages of delivery.

    3. Where the Supervising Trainer indicates that there is a 'Development Opportunity', please provide more detailed feedback in the comment box provided.

    4. After the Assessment of the course candidates, please provide a recommendation, review the form and then click 'Submit'

    5. Member Education staff will then review and support the Propitiatory Trainer on the next steps to becoming endorsed.

  • You can access the form multilabel times throughout a course to capture or update feedback at different point in time. To do this:

    1. Start a from for the Probationary Trainer

    2. Click Save once you have added/updated the form

    3. You will receive an email from (noreply@formresponse.com) with the subject line of 'Probationary Trainer Supervision Form'

Purpose

This form is to be used through a course in which a Probationary Trainer is delivering a course, to capture feedback from a Supervising Trainer. It is used to support someone undertaking the Probationary Trainer pathway and their journey to becoming an Endorsed Trainer.

It is to be used and completed in line with the SLSSA TAF Endorsement Policy and Procedure.
